I, JEANNE ROBERT B. CALIXTO, of legal age, Filipino, resident of No. 39 APA Compound, Philand Drive, Barangay Pasong Tamo, Tandang Sora Avenue, Quezon City, after having been duly sworn in accordance with law, freely and voluntary, without undue pressure from anyone or fear of intimidation and by way of affidavit do hereby depose and state that:
While on board a jeepney bound for SM North last Sunday, 2 April 2006, at around 7:00 to 7:30 pm, the jeep was help-up at the University Avenue (UP Diliman) corner Commonwealth Avenue;
The four hold-up men were armed with knives, an icepick and a gun, and because of freight and fear for my life, I did not resist when they collected our wallets and cellphones;
That I am an employee of Pentathlon Systems Resources Inc., and in my wallet were my office identification card and other cards I-Bank ATM, Medserv, Dental Health, BIR, Philhealth issued to me in connection with my employment thereto;
That I am executing this affidavit to be able to secure replacement for the cards I lost;
Affiant further sayeth naught.

Now I know kung bakit hirap na hirap akong mag ipon ng pambili ng cellphone ko at kung bat paulit ulit akong sinasabihan ng mga friends ko na bumili ako ng hiwalay na lalagyan ng mga cards (ids, atm & others) para daw pag nawala ang wallet ko eh wala masyadong hassle.

Waaa if i know itatapon lang naman nila yung wallet kasi pera lang ang kailangan nila bat kasi di nalang pera ang hiningi e. Kainis andun pa yung mga so treasured pix ko with old friends. Okay lang yung pera kasi kikitain naman yun kaso kasi yung mga money cant buy na stuff sa loob eeeeee!!!
